70대 여성 환자입니다. 6-7년 전 위암 진단을 받았으나 수술을 받지 않으신 분입니다 (과거 사진은 안 가지고 오심). 6개월 전 복통이 있어 내원하여 검사를 받았고 수술을 권유하였으나 거부하셨습니다. 최근 obstruction 증상으로 내원하여 수술을 권유하였고 다행스럽게 환자가 수술을 받아들이셨습니다. 아래의 결과였습니다.

Advanced gastric carcinoma
1. Location : middle third, Center at body and lesser curvature
2. Gross type : Borrmann type 3
3. Histologic type : tubular adenocarcinoma, poorly (poorly cohesive) differentiated
4. Histologic type by Lauren : mixed
5. Size : 6x6 cm
6. Depth of invasion : invades serosa (pT4a)
7. Resection margin: free from carcinoma, safety margin: proximal 4 cm, distal 2.5 cm
8. Lymph node metastasis : no metastasis in 49 regional lymph nodes (pN0), (0/49: "1", 0/0; "2", 0/8; "3", 0/6; "4", 0/11; "4sb", 0/1; "5", 0/0; "6", 0/3; "7", 0/6; "8a", 0/6; "9", 0/4; "11p", 0/0; "12a", 0/4)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : present
12. Peritoneal cytology : negative
13. AJCC stage by 7th edition: pT4a N0
그나마 다행입니다. 이 정도도 무척 운이 좋은 경우입니다.
