2015년 환자입니다. 40대 중반 남성입니다. 과음 후 발생한 복통으로 내시경 검사를 받으신 후 의뢰되었습니다. 어떻게 생각하십니까?

위각 후벽의 뚜렷한 암병소가 있었고 위체부 소만에도 두 개의 작은 궤양이 있었습니다. 사이의 점막도 모두 찌글찌글 했습니다. 함몰부 세곳에서 조직검사를 했는데 모두 암으로 나왔습니다. 수술을 하였습니다.

최종 병리결과입니다. Multiple synchrounous cancer가 아니라 큰 하나의 암이였습니다. 깊이는 조기위암에 해당하였는데 림프절 전이는 현저하였습니다. 정의 상 superficial spreading type이라고 할 수 밖에 없지만 상당히 aggressive한 것 같습니다. 3번은 문론 9번 (celiac) node까지 전이가 있었습니다. EB virus (-), c-erbB-2 (-)

1. Location : middle third, Center at body and lesser curvature
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, poorly (solid) differentiated with signet ring cell component (20%)
4. Histologic type by Lauren : mixed
5. Size : 9x5 cm
6. Depth of invasion : invades submucosa (sm1) (pT1b)
7. Resection margin: free from carcinoma, safety margin: proximal 2.5 cm, distal 5.5 cm
8. Lymph node metastasis : metastasis to 8 out of 57 regional lymph nodes (pN3a) (perinodal extension: absent) (8/57: "3", 6/13; "4", 0/7; "5", 0/1; "6", 0/11; "7", 0/3; "9", 2/10; "8a", 0/5; "11p", 0/2; "12a", 0/4; "4sb", 0/0; "1", 0/1)
9. Lymphatic invasion : present
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Peritoneal cytology : negative
