IIa + IIc는 점막하 침윤으로 나오는 경우가 많아서 늘 걱정합니다. 아래와 같은 병소를 보면 여러분은 ESD를 시도해 보시겠습니까? 저는 최대한 일정을 당겨서 ESD를 시행하고 있습니다. 김영란 법 때문에 일정 조정을 맘대로 할 수 없지만, 그래도 IIa + IIc 형 위암의 경우는 하루라도 빨리 당겨서 시술하려고 노력하고 있습니다.
50대 남성입니다. 최대한 일정을 당겨서 ESD를 시행하였습니다.
1. Stomach, ESD :
Early gastric carcinoma
1. Location : antrum, posterior wall
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 14 mm (2) vertical diameter, 11 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 4 mm, proximal 8 mm, anterior 14 mm, posterior 12 mm, deep 500 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
다행스럽게 결과는 좋았습니다. 법보다는 환자의 안전과 좋은 치료 결과가 우선입니다.
