저는 diagnostic ESD를 좋아하지 않습니다. 그러나 가끔 시행합니다. 조직형이 애매하다는 이유로 시행하지는 않습니다만, depth of invasion이 애매한 경우는 환자의 요청 혹은 동의(수술할 경우로 나오면 반드시 수술한다는 약속)가 있으면 시행하기도 합니다.
타 의료기관에서 ESD와 수술 중 어느 하나를 확실히 환자에게 권하지 못하여 찾아오신 환자였습니다. 타 의료기관에서 셜정하지 못한 것을 제가 결정할 수는 없는 일입니다. 최대한 자세히 설명하고 환자의 선택을 존중할 수 밖에 없습니다. 이러한 과정을 통하여 ESD 후 최종 병리결과 의거 수술 필요 여부를 결정하는 것으로 환자와 논의하였고 ESD를 하였고 수술이 필요한 것으로 나왔습니다.

Early gastric carcinoma
1. Location : low body, anterior wall
2. Gross type : EGC type IIa
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 16 mm (2) vertical diameter, 10 mm
6. Depth of invasion : invades submucosa, (depth of sm invasion : 2000 ㎛) (pT1b)
7. Resection margin : free from carcinoma(N) safety margin : distal 7 mm, proximal 5 mm, anterior 10 mm, posterior 16 mm, deep 100 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : present
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
