조직검사에서 'tubular adenoma high grade dysplasia (the possibility of adenocarcinoma cannot be completely excluded)' 소견으로 내시경 시술을 권유받고 의뢰되신 분입니다. 1년 전 급성 심근경색증의 병력이 있었습니다. ESD를 시도해 보시겠습니까?
2cm를 초과하는 IIa+IIc lesion이고 fold까지 끌려오는 양상인지라 점막암일 확률이 낮다고 판단되어 수술을 권했습니다.

3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size : 2.3x1.8 cm
6. Depth of invasion : invades submucosa (sm3) (pT1b)
7. Resection margin: free from carcinoma
8. Lymph node metastasis : no metastasis in 46 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Peritoneal cytology : negative
13. AJCC stage by 8th edition: pT1b N0
아무리 급성 심근경색증의 병력이 있어도 수술해야 할 환자는 과감히 수술을 보내고 있습니다.
