1. RS junction schwannoma

검진 내시경 발견 RS junction SMT로 low anterior resection 시행함.

2. Barrett adenocarcinoma mimicking sentinel polyp

검진 내시경에서 sentinel polyp 의심으로 조직검사를 하였는데 atypical gland로 나와 추적검사를 통하여 adenocarcinoma로 진단되었습니다. Barrett adenocarcinoma로 판단되어 ESD를 시행하였습니다 (민병훈 교수님).

6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 8 mm, proximal 9 mm, anterior 12 mm, posterior 14 mm
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent

아무리 sentinel polyp 같아도 처음 발견되었거나 과거와 모양이 다르다면 조직검사를 해야 합니다.

3. Ulcerative colitis

진단은 명확한데 scoring을 어떻게 할 것인가에 대하여 약간의 논의가 있었습니다. 현재 삼성서울병원에서는 Mayo score와 UCEIS를 모두 기술하고 있습니다. 김영호 교수님께서는 "(1) Mayo 0과 1은 remission으로 간주되는 수준이다, (2) 이 환자에서는 Mayo 2로 주는 것이 적당하겠다, (3) UCEIS가 보다 객관적이므로 Mayo score는 점점 그 중요성이 떨어지고 있다"는 의견을 주셨습니다.
