위식도접합부암 증례를 식도암인지 위암인지 구분하기는 쉽지 않습니다. 과거에는 squamous cell carcinoma면 식도암, adenocarcinoma면 위암으로 생각했습니다. 최근에는 Barrett adenocarcinoma가 식도암이므로 조금 더 어렵습니다.
아래 증례는 식도암일까요, 위암일까요?
Squamous cell carcinoma, gastroesophageal junction:
1) size: 6x3 cm
2) depth of invasion: perimuscular advantitia
3) Lymphatic invasion: not identified
4) Venous invasion: not identified
5) Perineural invasion: not identified
6) resection margin: free from carcinoma (proximal, 2 cm; distal, 10 cm)
7) Lymph node metastasis: metastasis to 5 out of 60 regional lymph nodes, (5/6 : lesser curvature, 3/18; greater curvature, 0/8; "periesophageal LN", 0/5; "G1", 0/8; "R9", 0/7; "5", 0/1; "7", 0/6; periesophageal, 2/7)
저는 이 증례를 식도암으로 분류하였습니다. 아직까지 squamous cell carcinoma인 위식도접합부암은 일단 식도암으로 간주하고 있습니다.
하부식도 혹은 위식도접합부 식도암은 위 주변 림프절로 전이할 수 있습니다. 이 환자에서는 lesser curvature 쪽으로 전이가 있었습니다.
