위각의 소만과 후벽을 involve하는 signet ring cell carcinoma로 EGC IIb에 해당하는 환자입니다. 첫 내시경 검사 10일 후에 제 외래를 방문하여 즉시 내시경검사를 시행하였습니다. 첫 내시경검사에서 flat (or very slightly depressed) pale irregular mucosa로만 관찰되던 병소가 외래오신 당일 시행한 내시경 검사에서는 중심부에 얕은 화산과 같은 모양을 보였습니다. 조직검사에 의한 변화로 해석할 수 있는 모양입니다.
3차 의료기관에서 근무하는 젊은 의사들은 다른 병원에서 진단받고 오시는 조기위암환자들을 검사하게 되는 경우가 많습니다. 오늘의 증례처럼 조직검사 후 모양이 크게 바뀌는 병소가 적지 않습니다. 의뢰된 환자를 검사할 때에는 첫 내시경 사진을 유심히 들여다보는 습관을 권합니다.

safety margin: proximal, 2.3 cm; distal, 2.9 cm
8. Lymph node metastasis : no metastasis in 36 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10.Venous invasion : not identified
11.Perineural invasion : not identified
12.Associated findings : ulceration (ul II)
