50대 여성입니다.
일견 위각부 위암으로 판단되지만 자세히 보면 위각부터 들문까지 소만을 따라 발적된 부위가 있고 표면이 불규칙하였습니다 (노란색 화살표). 근위전정부부터 들문까지 소만을 따라 매우 큰 위암이었습니다. 융기된 부위는 별로인데 signet ring cell carcinoma이면서 diffuse하게 침윤되어 있고 림프절 전이가 심한 상태였습니다.

5. Size : 12x4 cm

7. Lymph node metastasis : metastasis to 23 out of 24 regional lymph nodes (pN3b)
8. Lymphatic invasion : present (+++)
9. Venous invasion : present(intra- and extramural) (++)
10. Perineural invasion : present
