과거 Whipple 수술로 partial gastrectomy가 되어 있는 분입니다. 고도선종으로 의뢰되었으나 암의 가능성이 높다고 판단되었고 ESD를 했습니다. 역시 위암이 나왔습니다.
Stomach, endoscopic submucosal dissection:
Early gastric carcinoma
1. Location : high body, posterior wall
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 18 mm (2) vertical diameter, 17 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N)
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
일견 궤양이 있어 보이는데 병리선생님은 microscopic ulcer 없음으로 결과를 주셨습니다. 궤양 여부의 판단은 참 어렵습니다.
