함몰형 위암은 단기 PPI 사용으로 그 모양이 상당히 달라질 수 있습니다. P/D adenocarcinoma로 오신 분인데 첫 내시경 6일 후 재검을 하게 되었는데 악성 궤양 부위가 상당히 얕아졌고 fold 변화는 더욱 명확해진 상태였습니다. 대학 병원에서 내시경을 하시는 분들은 첫 내시경 소견이 어떠하였는지를 유심히 살펴보는 것이 좋습니다. 그리고 함몰부 크기가 작아졌다는 이유로 쉽게 benign으로 단정하면 안됩니다. 대부분의 환자가 PPI를 드시고 오기 때문입니다. 이 환자도 rabeprazole 20mg daily 복용하고 계셨습니다.
아래는 과거에 한번 소개한 증례인데요... 이보다 훨씬 심하게 (?) 호전된 궤양형 위암도 있었습니다.

1. Location : lower third, Center at body and lesser curvature
2. Gross type : EGC type IIc and IIa
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size : 3.9x2.5 cm
6. Depth of invasion : invades mucosa (muscularis mucosae) (pT1a)
7. Resection margin: free from carcinoma, safety margin: proximal 3.5 cm, distal 6.3 cm
8. Lymph node metastasis : no metastasis in 65 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 7th edition: pT1a N0
