Diffuse large B cell lymphoma인데 매우 긴 조직검사 결과를 받았습니다. 일단 옮깁니다.

CD 10 (CALLA) : Negative
CD 20 L26 : Positive in tumor cells
Bcl-6 : Positive in tumor cells (90 %)
Ki-67 : Positive in 95 % of tumor cells
MUM-1(IRF-4) : Positive in tumor cells (30 %)

Conclusion) Immunoprofiles are consistent with non-GCB type by Hans algorithm.

Method: FISH analysis was performed on formalin-fixed, paraffin-embedded tissue using Vysis LSI® C-MYC dual color, break apart probe (Abbott Molecular) and 50 non-overlapping nuclei were counted. The cutoff value for positivity is 3% for break apart probe.
Comment: C-MYC은 8q24에 위치합니다. Burkitt lymphoma는 90% 이상에서 C-MYC translocation이 있으며 대부분 IgH(14q32)와의 fusion입니다. Diffuse large B-cell lymphoma에서는 C-MYC translocation이 5-14% 있으며 IgH보다 IgL 이나 non-IG gene에 fusion된 경우가 많습니다. C-MYC과 BCL-2 모두 translocation이 있는 경우 ‘double hit lymphoma’라고 하며, 훨씬 나쁜 예후를 보입니다. 이 검사는 C-MYC의 translocation 여부만 확인하므로, fusion partner gene이 어떤 것인지는 알 수 없습니다. Comment: BCL-2는 18q21에 위치하며 follicular lymphoma의 75~85%에서 BCL-2/IgH translocation이 일어납니다. Diffuse large B-cell lymphoma의 18-20%에서도 Bcl-2의 translocation이 있으며, germinal center B-cell subtype의 경우 BCL-2의 translocation이 있으면 나쁜 예후를 보인다는 보고가 있습니다. BCL-2와 C-MYC 유전자 모두 translocation이 된 경우 ‘double hit lymphoma’라고 하며, 훨씬 나쁜 예후를 보입니다. 이 검사는 BCL2의 translocation 여부만 확인하므로, fusion partner gene이 어떤 것인지는 알 수 없습니다. (Visco, Haematologica 2013 98:255)

Method: FISH analysis was performed on formalin-fixed, paraffin-embedded tissue using Vysis LSI BCL6 dual color, break apart probes (Abbott Molecular).
BCL-6는 염색체 3q27에 위치하며 diffuse large B-cell lymphoma의 25%, Nodular lymphocyte predominant Hodgkin lymphoma의 50%에서 translocation이 있다고 알려져 있습니다. BCL-6 translocation의 partner gene은 절반이 IgH 등의 immunoglobulin gene이고 나머지는 다양한 non-Ig gene입니다. BCL6와 C-MYC에 모두 translocation이 있는 경우 ‘double hit lymphoma’라고 하며 훨씬 나쁜 예후를 보입니다. 이 검사는 BCL-6의 translocation 여부만 확인하므로, BCL-6의 fusion partner gene이 어떤 것인지는 알 수 없습니다.
