육안소견에서 함몰부가 뚜렷하여 수술한 환자 중 lamina propria까지만 침윤된 것으로 나오는 환자가 있습니다. 그러나 함몰부가 이 정도로 뚜렷하면 ESD를 시도하기보다는 수술을 권하고 있습니다. 물론 aggressive한 내시경 의사는 PPI를 투여한 후 ESD를 시행하기도 하는 것으로 알고 있습니다. 저는 되도록 absolute indication을 지키고 있습니다

Early gastric carcinoma (arising in an adenoma)
1. Location : lower third, Center at antrum and greater curvature
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, well differentiated
4. Histologic type by Lauren : intestinal
5. Size : 1x0.5 cm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin: free from carcinoma, safety margin: proximal 11.9 cm, distal 5.3 cm
8. Lymph node metastasis : no metastasis in 23 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Pre-existing adenoma : adenoma with high grade dysplasia and low grade dysplasia, depressed, 1.3x0.8 cm, negative resection margin
13. AJCC stage by 7th edition: pT1a N0
