타 병원에서 내시경 치료(ESD) 후 deep SM invasion으로 수술을 권유받고 본 병원을 방문하여 수술을 받으셨습니다. No residual tumor로 나왔습니다.
아래는 ESD pathology입니다. 저 같아도 ESD 했을 것 같습니다. 위체상부 병변은 생각보다 조금 깊다고 나오는 경우가 많습니다. 그래도 어쩔 수 없이 ESD를 합니다. 이 정도 가지고 처음부터 total gastrectomy를 권하기는 쉽지 않습니다.
Stomach, endoscopic submucosal dissection:
Early gastric carcinoma
1. Gross type : EGC type IIb
2. Histologic type : tubular adenocarcinoma, moderately differentiated with clear cell change
3. Histologic type by Lauren : intestinal
4. Size of carcinoma : (1) longest diameter, 10 mm (2) vertical diameter, 9 mm
5. Depth of invasion : invades submucosa, (depth of sm invasion : 2000 um) (pT1b)
6. Resection margin : free from carcinoma(N)
7. Lymphatic invasion : not identified(N)
8. Venous invasion : not identified(N)
9. Perineural invasion : not identified(N)
10. Microscopic ulcer : absent
11. Histologic heterogeneity: absent
