조기위암 내시경 치료를 위하여 상세히 문진하던 중 최근 심근경색증을 앓았음을 확인하였습니다. 검진에서 위암을 진단 받고 며칠 후 병원을 예약하였는데 하필 그 사이에 심근경색증을 앓아 인근 병원에서 치료를 받고 심장 stent까지 하셨다고 합니다. Dual platelet을 사용하고 있었는데요, 심장내과와 상의하여 ESD를 3개월 정도 미루고 그 기간 dual platelet을 사용하였습니다. 심장내과 권유대로 antiplatelet을 잠시 조정한 상태에서 ESD를 시행하였습니다.

5. Size of carcinoma : (1) longest diameter, 10 mm (2) vertical diameter, 9 mm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 12 mm, proximal 10 mm, anterior 20 mm, posterior 10 mm
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
환자는 처음에 위암 이야기만 하고 심근경색증 이야기는 하지 않으셨습니다. 상세한 문진 과정에서야 알게 되었습니다. 아무리 외래가 바빠도 물어볼 것은 물어보아야 하겠습니다. 빠른 진료보다는 바른 진료가 중요하니까요.
