50대 남성 위암으로 내시경 (조직검사 결과: M/D adenocarcinoma)과 CT 검사(stomach 주변에 작은 lymph node 들이 있음) 소견을 바탕으로 "병변이 일반적인 내시경 치료의 적응증보다는 다소 넓으며 CT에서 병변 주위에 작은 림프절이 보이는데 크기는 작으나 병변 주위에 위치해 있어서 림프절 종대의 원인이 암일 가능성도 배제할 수 없어서" 수술이 권유되었습니다.

수술 후 최종 병리결과는 아래와 같았습니다. 세포형은 첫 조직검사와 달리 미분화 혼재형이었으며 CT에서 의심된 바와 같이 위체하부 대만의 림프절 (4번 node입니다)에 전이가 있는 것으로 나왔습니다.

7. Resection margin: free from carcinoma, safety margin: proximal 3.5 cm, distal 3.5 cm
8. Lymph node metastasis : metastasis to 1 out of 54 regional lymph nodes (pN1) (see note) (perinodal extension: absent) (1/54: "3", 0/12; "4", 1/12; "5", 0/0; "6", 0/10; "7", 0/1; "9", 0/4; "8a", 0/6; "11p", 0/3; "12a", 0/4; "4sb", 0/0; "1", 0/2)

만약 이 환자를 ESD로 치료했다면 어떻게 되었을까요? 미분화 혼재암이 점막하 침윤을 보이거나, 점막하암이더라도 병변의 크기가 2-3cm를 넘는 경우에는 반드시 수술이 필요하다는 점을 고려하면, lymphatic invasion이 없고 SM1 cancer였지만 당연히 수술을 권해야 했겠지요.
위암을 진료하는 의사는 abdominal lymph node station을 알고 있으면 좋습니다. 외과 의사들과 communication에도 유용하고, 수술 결과를 이해하기도 좋습니다.
