위 전정부 조기위암으로 내시경치료를 하였습니다. 시술 도중 천공이 있어서 clipping을 하였습니다. 항생제를 사용하면서 3-4일 금식 후 식사를 시작하였는데 별 문제는 없었습니다.

치료 과정에서 환자가 가장 불편해 했던 것은 IV TPN이었습니다. 하얀색 TPN 주사 때문에 혈관이 붓고 아팠던 것입니다. 과연 IV TPN이 꼭 필요했던 것일까요? 1주일 이내의 금식이 예상되는 경우는 특별한 고칼로리 수액이 아니라 DNK2면 충분하지 않았을까요?

제가 내과 training을 받을 때 늘 함께하였던 Washington manual을 다시 들쳐보아야 할 것 같습니다. 요즘은 늘 과잉 치료가 문제입니다.
