Proximal gastrectomy 후 E-G stomy를 하면 역류 증세가 심하여 환자의 삶의 질에 지대한 영향을 미칩니다. 따라서 최근에는 proximal gastrectomy 대신 total gastectomy를 하는 경우가 많습니다.
위를 일부 남기기 위해서는 proximal gastrectomy 후 jejunal interposition이나 double tract reconstruction을 해서 식도와 위가 바로 만나지 않게 하면 좋습니다. 저희 기관에서는 double tract reconstruction (DTR)을 선호하고 있는데요... 문제는 total gastectomy 후 E-J stomy와 double tract reconstruction 후 E-J stomy가 거의 비슷해 보이므로 내시경의사가 remnant distal stomach을 관찰하지 않는 경우가 있다는 것입니다. DTR 수술인 것을 알고 remnant distal stomach을 관찰하려고 해도 접근이 어렵거나 anatomy가 헷갈리는 경우도 많습니다.
DTR 수술 환자의 내시경 검사를 증례로 소개하겠습니다.
Stomach, proximal gastrectomy:
. Early gastric carcinoma
1. Location : upper third, Center at cardia (Siewert II) and lesser curvature
2. Gross type : EGC type IIc+IIa
3. Histologic type : tubular adenocarcinoma, poorly differentiated
4. Histologic type by Lauren : intestinal
5. Size : 1.9x1.2 cm
6. Depth of invasion : invades mucosa (muscularis mucosae) (pT1a)
7. Resection margin: free from carcinoma, safety margin: proximal 0.3 cm, distal 12.3 cm
8. Lymph node metastasis : no metastasis in 47 regional lymph nodes (pN0) (0/47: "1", 0/0; "2", 0/5; "3", 3/21; "4", 0/3; "8a", 0/4; "7", 0/4; "9", 0/2; "11p", 0/3; "12a", 0/5)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 7th edition: pT1a N0
