양성 위궤양이 위암이 되는 것은 아닙니다. 그러나 위궤양과 위암의 위험인자는 겹치는 부분이 많습니다. 따라서 분명히 양성 위궤양이었던 분이 수년 후 위암환자로 다시 방문하는 경우가 있습니다.

위체부 후벽의 양성 위궤양이었고 치료는 잘 되었습니다. 한동안 안 오시다가 5년만에 다시 위암으로 내원하셨습니다. 사진을 잘 검토해보니... 과거 위암보다 몇 cm 아래, 소만쪽으로 함몰형 위암이 있었습니다. 과거 양성 위궤양 부분은 매우 깨끗한 scar로 관찰되었습니다. 외래에서 환자에게 그림을 그려가며 설명하였습니다.

수술을 시행하였고... 의외로 깊었습니다. PM cancer였습니다. 다행히 림프절 전이는 없었습니다.

4. Histologic type by Lauren : diffuse
5. Size : 3x1.5 cm
6. Depth of invasion : invades muscularis propria (pT2)
7. Resection margin: free from carcinoma, safety margin: proximal 2.3 cm, distal 9.4 cm
8. Lymph node metastasis : no metastasis in 43 regional lymph nodes (pN0)
9. Lymphatic invasion : present
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 7th edition: pT2 N0
