2013년 환자를 다시 생각해 보았습니다.

위암 환자입니다. GIST로 추정되는 병소도 함께 있었습니다. 위암 자체만 따지면 ESD의 적응증이었습니다. 그런데 5 cm가 넘는 GIST가 있어서 어떻게 치료할지 고민이었습니다. 위암은 ESD 하고 GIST는 wedge resection을 할 수도 있고, 둘을 한꺼번에 subtotal gastrectomy로 치료하는 것도 고려할 수 있는 상황이었습니다. 여러분은 어떻게 하시겠습니까?

저는 GIST가 작지 않다는 점을 고려했습니다. 수술로 한꺼번에 치료하였으며 아래의 결과였습니다.

Stomach, subtotal gastrectomy:
I. Early gastric carcinoma
1. Location : middle third, Center at body and lesser curvature
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, well differentiated
4. Histologic type by Lauren : intestinal
5. Size : 1.3x1.0 cm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin: free from carcinoma, safety margin: proximal 2.5 cm, distal 9 cm
8. Lymph node metastasis : no metastasis in 57 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Peritoneal cytology : negative
13. AJCC Stage by 7th edition: T1a N0
II. Gastrointestinal stromal tumor of intermediate risk of malignant potential by NIH (2002) and low risk (3.6%) of progressive disease by Miettinen(2006):
1) tumor size: 6x5.5x5 cm
2) mitosis: 5/50 HPF (high powered fields)
3) histological type: spindle
4) necrosis: present
5) cellularity: high
6) cellular atypia: moderate
7) invasion into mucosa: absent
8) resection margin involvement: absent

5년 지난 시점에 다시 생각해보지만 아쉬운 점은 없습니다. GIST가 작았으면 ESD + wedge resection을 고려할 수 있었겠지만, 5cm가 훨씬 넘는 GIST이므로 한꺼번에 치료하는 것도 선택할 수 있는 option이라고 생각합니다. 지금도.
