몇 년 전 환자입니다. 70대 여성이었습니다. Gastric dysplasia라는 조직진단으로 ESD 시도하였으나 범위가 넓어 외과로 전과되었다가 2차 의견을 위하여 의뢰된 환자였습니다.
재검을 하였는데 일단 조직진단은 둘 다 암으로 나왔습니다.
다소 경계가 불분명하지만, 상대적으로 뚜렷하게 융기된 부위는 ESD 적응증에 해당한다고 판단되어 두 개의 ESD를 동시에 진행하였고 결과는 아래와 같았습니다. 5년 지난 현재까지 재발없이 잘 지내고 계십니다. 늘 힘들지만 그래도 이런 환자를 보면 ESD를 전공한 보람을 느낍니다.

6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N)

10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
