IIa+IIc형으로 경계가 다소 모호했습니다. 경계가 모호한 위암의 경우 이런 저런 특수 검사법을 사용하여 치료 범위를 결정할 수 있습니다. 개인적으로는 확대내시경에 관심을 가지고 있습니다. 하지만 경험상 ESD를 조금 넓게 해버리면 만사 깔끔한 것 같습니다.
GC of proximal antrum, biopsy(ESD) :
. Early gastric carcinoma
1. Location : antrum, greater curvature
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, well differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 8 mm (2) vertical diameter, 8 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 10 mm, proximal 10 mm, anterior 12 mm, posterior 14 mm, deep 500 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
