Cardia 병소입니다. 조직검사에서 atypical glands, suspected high grade dysplasia로 의뢰되었으나 내시경 육안소견에서 암이 의심되어 위암에 준한 ESD를 시행하였습니다. 특히 하부식도에서 병소를 관찰할 때 squamocolumnar junction의 일부가 약간 융기되어 있어서 식도를 일부 침범한 것으로 판단되었습니다. 하부식도를 포함한 ESD를 하였고 stricture를 막기위한 preemptive steroid를 투여하였습니다.

7. Resection margin : free from carcinoma(N), safety margin : distal 4 mm, proximal 3 mm, anterior 6 mm, posterior 8 mm, deep 1100 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
