Endoscopic oncology를 전공하는 입장에서 위암 내시경 치료 후 재발하지 않고 10년간 잘 지내시는 분들을 볼 때마다 가슴이 벅차옵니다. 조기위암 내시경치료 후 재발률이 매우 낮다지만, 그래도 늘 감격스럽습니다.

오늘의 증례를 소개드린 이유는 아래 사진 때문입니다. 10년 전에는 insulated tip이 없는 그냥 needle knife로 시술을 했습니다. 행여 천공이 될까봐 조마조마해 하면서 시술했던 기억입니다. 지금은 IT-knife, Dual knife 등 훨씬 안전한 도구들이 있어서 천공 발생률이 낮아졌습니다. 단지 마음대로 쓰지 못할 뿐입니다.

우리 의료진들은 누구나 새로운 기구를 개발하고 술기를 닦아 안전한 시술이 될 수 있도록 애써야 합니다. 하물며...... 있는 기구조차 마음대로 쓰지 못하게 제한하는 어처구니 없는 정부 규정은 하루 빨리 고쳐져야 할 것입니다. 특별한 경우를 제외하고는 ESD를 할 때 knife를 하나만 쓰도록 규정하고 있는 현행 제도는 환자를 위험에 빠뜨리고 있습니다. 우리나라 의료제도는 박정희 독재정권의 아이디어에서 벗어나지 못하고 있습니다. 통제, 통제, 통제...
