저도 선종으로 의뢰되신 분입니다.
소작술 (APC ablation) 가능성 고려한 상태에서 일단 외부 슬라이드 재판독을 의뢰하였는데 고도 선종으로 보고되었습니다. 병리 의사 사이의 관찰자간 차이입니다. 저는 저희 병리과 선생님의 판단에 의존하고 있습니다. 저도 선종은 몇 가지 조건을 만족하면 소작술을 선택할 수 있으나, 고도 선종은 원칙적으로 ESD를 하고 있습니다. 따라서 환자에게 아래와 같이 치료방법 변경을 설명드렸습니다.
ESD를 하였고 결과는 아래와 같았습니다. 치료방법을 변경이 적절하였던 것으로 판단됩니다.

2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 20 mm (2) vertical diameter, 8 mm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 14 mm, proximal 10 mm, anterior 8 mm, posterior 10 mm, deep 1700 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
