2015년 환자입니다.
고도선종 ESD 후 위암으로 진단이 바뀌는 환자가 많습니다. 최소한 1/3입니다. 병소가 크면 클수록 암일 확률은 다소 높아집니다. 병원에 따라 다르지만 50% 이상 암으로 바뀌는 병원도 있습니다. 병리의사의 눈높이 차이가 중요 원인이라고 생각합니다.

1. Location : proximal antrum, lesser curvature
2. Gross type : EGC type IIa+IIc
3. Histologic type : tubular adenocarcinoma, well differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 52 mm (2) vertical diameter, 19 mm
6. Depth of invasion : invades mucosa (lamina propria) (pT1a)
7. Resection margin : free from carcinoma(N); safety margin : distal 9 mm, proximal 9 mm, anterior 8 mm, posterior 10 mm, deep 1800 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
