본 병원에서 내시경 재검을 시행한 의사가 저에게 질문을 하였던 환자입니다. “위암입니까? 식도암입니까?”

식후 명치 불편감으로 시행한 내시경에서 cardia의 종양 발견되었고 adenocarcinoma로 확인되었습니다. 첫 대학병원에서 위암 소견으로 위 전절제술 권유 받았으며, 두번째 대학병원에서는 식도암에 준한 수술이 필요하다고 들었습니다. 환자는 본 병원 흉부외과를 방문하였습니다.

외부 병원과 본 병원의 내시경 사진에서 병소의 epicenter가 EG junction 보다 아래쪽이면서 Barrett esophagus의 소견이 전혀 없어서 저는 위암이라고 대답을 하였습니다. 위암인데 식도쪽으로 침윤이 있는 상태입니다. Total gastrectomy + distal esophagectomy가 시행되었습니다.

Esophagus and stomach, total gastrectomy: Tubular adenocarcinoma, poorly differentiated, esophagogastric junction:
1) invasive carcinoma size: 3x3 cm
2) extension to periesophageal soft tissue (adventitia)
3) endolymphatic tumor emboli: present
4) perineural invasion: present
5) resection margins: free from carcinoma, safety margin: proximal, 2cm ; distal, 18cm ; circumferential (adventitial) margin(deep), 200㎛
6) metastasis to 6 out of 39 regional lymph nodes with extracapsular extension (size of the largest metastasis: 0.3 cm (6/39: "Gastric lymph node 4" for frozen section-1, 0/1; "common hepatic lymph node" for frozen section-2, 0/1; "8 distal lymph node", 0/1; "Common hepatic lymph node", 0/0; "celiac lymph node", 0/2; "3,5LN", 6/18; "4,6LN", 0/16)
7) treatment effect: no prior treatment
