[Gastric cancer 637 - 매우 이상한 모양의 multiple ulcer와 주변 점막의 nodularity]

Focal cancerous change in the large adenoma with HGD. Despite its bizarre morphology, there is no sign of deep invasion. Final pathology was EGC arising from adenoma.

매우 이상한 모양의 multiple ulcer와 주변 점막의 nodularity를 보인 병변이었습니다.

검사를 하신 선생님께서는 Advanced gastric cancer B-III로 impression을 주셨습니다.

- Location : LC of low body
- Size : 10 cm
- LC of body의 거의 전 벽을 replacement 하는 경계가 불분명한 병변이 관찰된다. hyperemicirregular한 표면이 관찰된다. 병변에 1.5~2cm 크기의 round shape의 ulcer를 동반한다. Biopsy는 angle에 있는 병변에서 부터 시행함, Bx #1 (LC of low body, 가장 angle side), Bx #2 (LC of low body), #3 (MB of low body, 가장 HB side).

조직검사 결과는 아래와 같았습니다.

#1. Ulcer with focal atypical glands
#2. Ulcer and atypical glands with high grade dysplasia
#3. Tubular adenocarcinoma, well differentiated

이 환자가 정말 AGC일까요? Bizarre 한 모양이지만 깊은 궤양도 아니고, mass effect도 없고, infiltration 양상도 아니라 EGC라고 생각되었습니다. 검사하신 선생님께 아래와 같은 메일을 보냈습니다.

"검사 잘 하셨습니다. Description도 나쁘지 않은데…. AGC였을까요? 저는 EGC에 걸겠습니다. 아이스크림 내기합시다."

최종 결과는 아래와 같았습니다.

2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, well, differentiated
4. Histologic type by Lauren : intestinal
5. Size : 2.3x1.0 cm (carcinoma component)
6. Depth of invasion : invades submucosa (sm1) (pT1b)
7. Resection margin: free
8. Lymph node metastasis : no metastasis in 22 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Pre-existing adenoma : adenoma with high grade dysplasia, 5.5x3.5 cm
13. Peritoneal cytology : atypical cell
14. AJCC stage by 8th edition: pT1b N0
