Pyloric ring의 암은 내시경 평가가 어렵습니다. 원래 pyloric ring이 좁은 공간이기 때문입니다. 넓직한 위체부나 전정부와는 사뭇 다르게 보이기 때문입니다. 아래 사진을 보시고 어느 정도인지 짐작해 보시기 바랍니다. 크기와 침윤 깊이 말입니다.
재검하고 수술하였습니다.
Stomach, subtotal gastrectomy: Advanced gastric carcinoma
1. Location : lower third, Center at pylorus and circle
2. Gross type : Borrmann type 3
3. Histologic type : tubular adenocarcinoma, poorly (poorly cohesive) differentiated
4. Histologic type by Lauren : diffuse
5. Size : 5.5x1.8 cm
6. Depth of invasion : invades muscularis propria (pT2)
7. Resection margin: free from carcinoma, safety margin: proximal 9.5 cm, distal 1 cm
8. Lymph node metastasis : metastasis to 3 out of 31 regional lymph nodes (pN2), (perinodal extension: absent), (3/31: "3", 0/10; "4", 0/6; "5", 0/1; "6", 0/2; "7", 3/4; "9", 0/0; "8a", 0/2; "11p", 0/5; "12a", 0/1; "4sb", 0/0; "1", 0/0)
9. Lymphatic invasion : present
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. Peritoneal cytology : negative
13. AJCC stage by 8th edition: pT2 N2
생각보다 크고 깊고 림프절 전이(5,6번이 아니고 7번이었습니다)까지 있었습니다. 크기는 5.5x1.8 cm로서 둥글지 않고 길쭉했습니다. 해부학적 이유 때문이겠지요.
정리합니다. Pyloric ring cancer는 보기보다 크고 깊고 림프절 전이가 있는 경우가 많습니다.
