50대 여성입니다. 조직검사가 P/D였습니다. 환자는 내시경치료를 원하셨다고 하는데요... 당연히 수술해야 하는 병입니다.

2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, poorly (solid) differentiated
4. Histologic type by Lauren : mixed
5. Size : 1.9x1.3 cm
6. Depth of invasion : invades submucosa (sm2) (pT1b)
7. Resection margin: free from carcinoma, safety margin: proximal 6.1 cm, distal 3.7 cm
8. Lymph node metastasis : no metastasis in 26 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 8th edition: pT1b N0

ESD는 환자가 원한다고 해 드리는 그런 시술이 아닙니다. 적응증을 잘 지키는 것이 최선입니다. 수술이 필요한 환자는 수술, ESD가 필요한 환자는 ESD. 타협은 없음.
