High grade dysplasia라는 조직검사 결과로 의뢰된 환자입니다. Olympus 내시경이었습니다. 그러나 함몰형 병소의 크기와 경계를 살펴보았을 때 위암의 가능성이 높다고 판단되었습니다.
ESD를 시행하였습니다. Pentax 내시경이었습니다.
ESD: Early gastric carcinoma
1. Location : antrum, lesser curvature-posterior wall
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 20 mm (2) vertical diameter, 15 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 7 mm, proximal 9 mm, anterior 8 mm, posterior 24 mm, deep 800 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
13. Associated finding: Gastritis cystica
비록 조직검사에서 high grade dysplasia가 나왔더라도 내시경 육안소견이 암이라고 판단되면 최종 결론은 대부분 암입니다. 적극적인 치료가 필요합니다.
