내시경에서 "On the cardia to LC of high body, ulceroinfiltrative lesion was noticed(Bx #1). On the GC of low body to mid body, there was seen hypertrophied rugae with erythematous changes and a few erosions. It's consistency was felt relatively hard, so random biopsy was done (Bx #2)."와 같은 소견으로 "1. AGC B III (#1) involving cardia and GE junction, 2. Hypertrophied rugae (#2) R/O AGC B IV"의 impression을 주었던 환자입니다.
1번 조직검사에서만 암이 확인되었고 total gastrectomy를 하였습니다. 수술 후 병리결과는 Borrmann type 4, tubular adenocarcinoma (poorly differentiated), diffuse by Lauren, 16.5x12 cm, invades serosa (pT4a), no lympho node metastasis in 52 regional lymph nodes (pN0)으로 나왔습니다.
결국 cardia의 병소부터 위체부의 병소까지 모두 diffuse infiltration을 보이는 Borrmann type IV였던 것이고 cardia 부분에 궤양이 있었고 그 부위에서만 수술전 조직검사에서 암으로 나왔던 것입니다. Undifferentiated cancer는 PET에서 uptake가 없을 수도 있는데 이 환자는 PET에서 약하지만 그래도 proximal stomach의 diffuse uptake가 있었습니다. 수술전 CT에서는 "gastric cardia와 upper body에 abnormal wall thickening이 있음. Stomach angle에서 proximal antrum 쪽으로도 infiltrative abnormal wall thickening이 의심됨"으로 판독되어 넓은 diffuse cancer의 가능성이 제기되었습니다.
궤양형 위암 환자에서 주변의 fold thickening이 있으면 전체가 Borrmann type IV일 수 있다는 사실을 잘 보여주는 증례라고 생각합니다.
