내시경집담회 20170504. EndoTODAY 이준행

1. Pyloric ring ulcer, Hp (+) and NSAID (-)

2. EG junction cancer (SCC) + stage IV lung cancer (adenocarcionma)

폐암과 식도 dysplasia로 의뢰된 분입니다. 폐는 stage IV adenocarcinoma로 확인되었습니다. 식도는 최근 조직검사 전 외부 내시경 사진을 구할 수 없어서 본 병원에서 재검한 사진을 모았습니다... 역류성 식도염 소견으로 보기에는 경계가 불규칙한 모양이었습니다. 조직검사에서 squamous cell carcinoma (M/D)로 확인되었습니다.

수술적 절제가 불가능한 병기의 폐암도 동시에 있었기 때문에 식도에 대한 치료 계획은 잡지 못하고 폐암에 대한 치료만 시행하였습니다. 식도 병소에 대해서는 Lugol spray, electrosurgical unit (ESU) 등 추가 평가가 필요하겠지만 폐암만 없었다면 ESD를 시행할 수 있지 않았을까 생각됩니다.... 여하튼 식도에 대해서는 추가 검사나 치료는 할 수 없었습니다.

경우는 조금 다르지만 아래 증례들도 유심히 살펴보면 좋을 것 같습니다. 보초용종의 내시경 진단에서 가장 중요한 점은 악성질환과의 감별진단이다. 간혹 들문암(cardia cancer)이 보초용종과 비슷한 모습을 보이는 경우가 있으므로 매우 주의해야 한다.

따라서 보초용종이 처음 발견되는 경우 조직검사를 통하여 암이 아니라는 점을 확인해야 하며 PPI 투여 후 추적관찰을 해 보는 것이 좋다.

3. EGC

위체부 소만의 경계가 불분명한 표재성 병소입니다. 조직검사는 adenoma with focal high grade dysplasia로 나와 ESD를 시행하였습니다. 위암은 작으나 pre-existing adenoma가 약 5 cm 정도 되었습니다. 첫 내시경에서 size underestimation 되었던 것으로 생각됩니다.

8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Pre-existing adenoma : tubular adenoma with high grade dysplasia; 1) gross type : flat, 2) size : 48x25 mm, 3) resection margin : free from adenoma(N)
12. Microscopic ulcer : absent
13. Histologic heterogeneity: absent

4. Colon cancer + EGC

대장암 수술 예정인 환자의 위내시경입니다. 위암의심이었으나 계속 암이 나오지 않아서 추적검사를 하였고 결국 3번째 조직검사에서 암으로 확인되어 수술을 하였습니다.

6. Non-exposed polypoid type ampullary cancer

30대 남자에게 황달이 오면 간염부터 생각하게 됩니다. 그러나 드물게 악성질환인 경우도 있습니다.

이 환자는 Ampulla of Vater cancer였으며 PPPD 수술을 하였고 아래와 같은 결과였습니다. 3년 후 multiple hepatic metastasis를 보였습니다.

(1) T1 : Tumor limited to AoV or sphincter of Oddi (extension to submucosa)
(2) Involvement of pancreas and duodenum : absent
(3) Extension to distal common bile duct : absent
(4) N0 : No regional lymph node metastasis (0/28: pericholedochal, 0/21; peripancreatic, 0/7)
(5) M0 : No distant metastasis
(6) Negative (pancreas, common bile duct, retropancreatic) resection marginsborder="1">

Non-exposed polypoid type ampullary cancer의 대표적인 예입니다.

일반 내시경 끝에 cap을 장착하면 십이지장 유두부를 조금 더 잘 관찰할 수 있습니다 (Abdelhafez M. Am J Gastroenterol 2017). 모든 환자에게 필요할 것 같지는 않고, 유두부를 자세히 관찰해야 할 필요가 있는 환자에서 적용해 볼 수 있을 것 같기도 합니다. 아예 처음부터 측시경을 사용하는 것도 한 방법이기는 합니다만.
