1. Achalasia

5년간 언친 증세가 있고 점차 체중이 빠졌던 분입니다. 과거 내시경 후 역류성 식도염으로 들은 바 있다고 합니다. 그러나, 내시경 사진을 자세히 보면 EG junction이 tight 하고 하부식도가 늘어난 모양입니다. 식도 증상이 잇는 사람에서 이 정도 소견이 보이면 achalasia를 의심할 수 있어야 합니다.

POEM으로 치료하였습니다 (민양원 교수님).

2. EGC

Stomach, subtotal gastrectomy:
Early gastric carcinoma
1. Location : lower third, Center at antrum and greater curvature
2. Gross type : EGC type IIa
3. Histologic type : tubular adenocarcinoma, poorly differentiated
4. Histologic type by Lauren : diffuse
5. Size : 1.7x1.4 cm
6. Depth of invasion : invades submucosa (sm2) (pT1b)
7. Resection margin: free from carcinoma, safety margin: proximal 5.2 cm, distal 7.4 cm
8. Lymph node metastasis : no metastasis in 31 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified

3. A colon LST

Ascending colon, distal, endoscopic submucosal dissection:
Tubulovillous adenoma ;
1) size: 2.6x1.6 cm
2) negative resection margins

4. BGU

추석 때 과음 후 속이 아프고 쓰렸다는 증상으로 내시경 검사

궤양약 및 제균치료 후 호전된 모습니다.
