2016년 증례입니다.
보통 보만 4형 진행성 위암은 매우 어렵습니다. 그런데 어떤 환자는 너무 쉽습니다. 내시경과 CT에서 그야말로 명확한 경우도 있기 때문입니다. 외부 조직검사와 본 병원 조직검사에서 모두 암이 나오지 않았지만 내시경 육안소견이 너무나 명확하여 바로 수술을 보냈습니다.
외래 당일 내시경 검사를 하였고, 다음 날 입원하여, 그 다음 날 외과로 전과하고, 그 다음 날 수술을 하였습니다. 지금까지 수술한 보만 4형 진행성 위암 중 가장 빨랐던 경우 아닌가 생각됩니다. Ultra-super-fast high-speed surgical treatment for biopsy-negative Borrmann type IV gastric cancer의 증례였습니다.^^ 다행스럽게 복막 전이나 림프절 전이는 없었습니다.

2. Gross type : Borrmann type 4
3. Histologic type : tubular adenocarcinoma, poorly (poorly cohesive) differentiated
4. Histologic type by Lauren : diffuse
5. Size : 18x18 cm
6. Depth of invasion : invades serosa (pT4a)
7. Resection margin: free from carcinoma
8. Lymph node metastasis : no metastasis in 41 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : present
12. Peritoneal cytology : negative
13. AJCC stage by 7th edition: pT4a N0
