첫 내시경 (조직검사 전)에서는 작은 함몰부였고 뚜렷한 궤양은 아니었고 fold 변화는 없었습니다. 그런데 의뢰 후 재검한 내시경에서는 뚜렷한 fold 변화가 생겼습니다. 처음부터 이처럼 뚜렷한 fold 변화가 보이면 내시경 치료를 시행하지 않는 것이 보통입니다. 그런데 처음에는 없던 뚜렷한 fold가 조직검사 후 보였다면 어떻게 하는 것이 좋을까요? 약간 tricky한 이슈입니다. 저는 처음 내시경 사진을 주의깊게 관찰하고 있습니다. ESD를 하였고 아래와 같은 결과였습니다.

1. Location : antrum, lesser curvature
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, well differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter,14 mm (2) vertical diameter, 8 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 11 mm, proximal 12 mm, anterior 16 mm, posterior 8 mm
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
