약간 넓은 EGC IIc였습니다. 그런데 첫 내시경 검사와 의뢰 후 재검의 내시경 모양이 사뭇 달랐습니다. 첫 내시경 검사에서는 공기를 많이 넣은 상태에서 사진이 찍혔고, 재검 후에는 공기의 양이 약간 작았기 때문일 수 있습니다. 아니면, 조직검사로 인하여 약간 deformity가 발생하였기 때문일 수도 있습니다. 여하튼 조직검사는 M/D adenocarcinoma였습니다. 어떤 치료방법을 선택하시겠습니까?

저는 조직검사로 인한 deformity 보다는 공기 팽창으로 인하여 첫 검사에서 fold 변화, 병소의 두께 등이 충분히 평가되지 못했던 것 아닌가 추정하였습니다. 수술을 의뢰하였습니다.

4번 node 전이가 있었던 점막하암이었습니다.

만약 이 환자가 ESD를 받았고 다음과 같은 병리결과를 보였더라면 어떻게 하는 것이 좋았을까요?

- Histologic type by Lauren : mixed
- Size : 4.5x3.5cm
- Depth of invasion : invades submucosa (sm1)
- Resection margin: free from carcinoma,
- Lymphatic invasion : not identified
- Venous invasion : not identified
- Perineural invasion : not identified

예. 그렇습니다. (1) 미분화암 혹은 혼재암이 점막하 침윤을 보이면 반드시 수술을 권하는 것이 좋습니다. (2) 분화형 암이라도 점막하암이 2-3cm를 넘으면 반드시 수술을 권하는 것이 좋습니다.

이 환자는 ESD를 했더라도 당연히 수술을 권해야 했던 환자였습니다. 수술의 이유는 림프절 전이 위험 때문이고, 이 환자는 실제로 림프절 전이가 있었습니다.
