병소의 proximal margin 평가과 심달도 예측이 어려웠던 환자입니다. Clipping을 하였는데 그 중 하나는 병소의 edge로부터 조금 떨어져 있었습니다. 내시경에서는 함몰부와 정상의 경계가 명확하지 않았는데 절제 표본에서는 훨씬 더 명확한 경우가 있는데, 이 증례도 그 중 하나입니다. 내시경 검사의는 진행성 위암을 예측하였는데 다행스럽게 수술 병리는 점막하층까지 침윤된 조기위암이었고 소만, left gastric, celiac node에 전이가 있었습니다.

아래도 비슷한 증례입니다. 오래 전 환자인지라 병리 결과 보고 스타일이 현재와는 많이 다릅니다. 수술로 절제한 표본의 함몰부 경계는 명확한데, 내시경에서는 그 경계가 불분명하였습니다. 그 결과 병소 내부에 clipping을 하였던 실수 증례입니다.

Signet ring cell carcinoma, diffuse type;
1) tumor size: 8.2x6.8 cm
2) extension to inner proper muscle layer
3) no endolymphatic tumor emboli
4) negative resection margins (proximal 2.7 cm; distal 4 cm)
5) no metastasis in 42 regional lymph nodes (0/42), (lesser curvature 0/15; greater curvature 0/17; "LN" for frozen 0/3; "7,9" 0/7; "8a" 0/0; "12" 0/1; "12a" 0/2)
