[Gastric cancer 562 - Atypical gland with HGD 과연 암임가?]

뚜렷한 양성 궤양이 아닌데 조직검사에서 atypical gland로 나오면 암일 가능성이 있다고 말씀드렸습니다. 조직검사에서 high grade dysplasia이면 ESD 후 암으로 진단이 바뀔 가능성이 최소한 1/3입니다.

병리과 의사가 atypical glands with HGD로 주면 거의 대부분 최종 진단이 암입니다. 병리과 의사가 "Atypical glands with high grade dysplasia, suggestive of TUBULAR ADENOCARCINOMA, WELL DIFFERENTIATED (foveolar type)"로 주면 거의 거의 거의 대부분 암입니다. 아래 환자의 조직검사가 그랬습니다. 그런데 이런 병리결과만 가지고 암으로 확진할 수 있을까요? 암 코드를 줄 수 있을까요? 99%는 100%인가요?

저는 암의 가능성이 매우 높다고 설명드렸습니다. 가능성이 매우 높다는 것이지 확실하다는 것은 아니라고 말씀드렸습니다. 그런데 환자분이 "암입니까?"라고 다시 묻더군요. 그래서 또 다시 설명한 후 그 내용을 의무기록에 남겼습니다.

ESD를 시행하였습니다. 예상한 그대로 암으로 나왔습니다.

'암 같은 것'은 '암 같은 것'이지 '암'은 아닙니다. 99%는 100%가 아닙니다. 100%일 때 암 code를 줍시다.
