조기위암 내시경 치료 후 SM invasion 소견이 있었습니다. 500um였습니다. 환자와 충분히 상의 후 추적관찰을 하는 것으로 결정하여 시술 2개월에 내시경 검사를 하였습니다. 추적 내시경 검사에서 artificial ulcer가 호전되었는데요 AHS stage로 무엇을 붙이는 것이 좋을까요?
ESD: Early gastric carcinoma
1. Location : body, anterior wall
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 6 mm (2) vertical diameter, 6 mm
6. Depth of invasion : invades submucosa, (depth of sm invasion : 500 ㎛) (pT1b)
7. Resection margin : free from carcinoma(N), safety margin : distal 12 mm, proximal 12 mm, anterior 14 mm, posterior 14 mm, deep 1,000 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
검사를 해 주신 선생님은 H1으로 붙이셨습니다. 그러나 ulcer crater가 다 닫힌 상태이므로 S1이 더 좋다고 생각합니다. H1은 덜 아문 상태이므로 궤양약을 더 드릴 것을 고려할 수 있지만, S1은 다 아문 상태로 보고 투약할 필요는 없을 것입니다.
두 가지 더 생각할 점
1. ESD 후 추적내시경에서 보이는 소견도 일반적인 소화성 궤양에 준하여 AHS 분류를 붙여야 할까요? 꼭 그럴 필요는 없지만 communication을 위하여 붙여두는 것이 편할 것 같습니다. 예를 들어 H stage면 약을 더 쓰면 좋겠다는 sign으로 삼을 수 있으니까요.
2. 만약 ESD 후 artificial ulcer에서 H1 stage까지 호전된 상태라면 꼭 PPI를 더 써야 할까요? 저절로 발생한 소화성 궤양에 비하여 더 잘 아물기 때문에 H1 stage이고 그 크기가 작으면 PPI를 쓰지 않아도 저절로 좋아지는 것 같습니다.
