ESD를 위하여 의뢰되었습니다. 제가 잘 아는 분의 가족인지라 치료 계획 결정이 부담되는 상황이었습니다. 게다가 수술을 한다면 total gastrectomy인지라... 여러분은 어떻게 하셨겠습니까?

아무리 아는 분의 부탁을 받았어도 치료 원칙은 동일합니다. 수술이 필요하면 과감히 수술을 권합니다. 수술을 하였고 아래 결과였습니다.
Stomach, radical total gastrectomy: Early gastric carcinoma
1. Location : upper third, Center at body and lesser curvature
2. Gross type : EGC type IIb
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size : 4.2x3.7 cm
6. Depth of invasion : invades submucosa (sm1) (pT1b)
7. Resection margin: free from carcinoma, safety margin: proximal 1.4 cm, distal 16.7 cm
8. Lymph node metastasis : no metastasis in 64 regional lymph nodes (pN0) (0/64: "2", 0/7; "3", 0/20; "4", 0/16; "5", 0/0; "6", 0/7; "7", 0/2; "9", 0/1; "8a", 0/0; "11p", 0/4; "12a", 0/4; "4sb", 0/0; "1", 0/0; "11d", 0/0; "mesentery LN", 0/1; "10", 0/0; "4sa", 0/0; perigastric, 0/2)

VIP인 것을 고려하고 치료 계획을 잡으면 잘못된 치료가 되기 쉽습니다. 이를 VIP syndrome이라고 합니다. VIP는 VIP가 아닌 것처럼 생각하고 진료해야 가장 좋은 결과가 나옵니다. Very important person이라고 생각하지 말고 very irritable person으로 생각하고 치료하면 모두가 만족하는 좋은 결과가 나옵니다.
