In the initial endoscopy at primary clinic, the cancer size was estimated as 1 cm. During the ESD, however, I found the tumor is much bigger than that. Size measurement is one of the important aspect of endoscopic examination, especially in the era of ESD.
1cm 크기의 위암으로 의뢰되었습니다. ESD를 시도하였는데 생각보다 커서 약간 놀랐지만 시술은 별 무리 없이 진행되었습니다. 과거에는 내시경 검사의 의미가 주로 위암 진단 자체였다면, 이제는 진단뿐만 아니라 치료방침 결정도 내시경 검사의 중요 요소입니다. 크기 평가는 빠질 수 없는 요소이지요. 정확한 크기를 기록에 남기기 위하여 노력해 봅시다.

1. Location : body, posterior wall
2. Gross type : EGC type IIa
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 30 mm (2) vertical diameter, 17 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 5 mm, proximal 5 mm, anterior 10 mm, posterior 8 mm, deep 500 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
