[Gastric cancer 632 - EGC IIb with fold convergence]

EGC IIb may have some converging folds at its periphery. In this case, loss of gastric fold at posterior wall of lower body, subtle mucosal change, and converging folds at its periphery are important clues.

Stomach, radical subtotal gastrectomy: Early gastric carcinoma
1. Location : [1] middle third, Center at low body and posterior wall
2. Gross type : EGC type IIb+IIc
3. Histologic type : tubular adenocarcinoma, poorly (poorly cohesive) differentiated
4. Histologic type by Lauren : mixed
5. Size : 4.7x3 cm
6. Depth of invasion : invades submucosa (sm2) (pT1b)
7. Resection margin: free from carcinoma, safety margin: proximal 0.8 cm, distal 7.8 cm
8. Lymph node metastasis : no metastasis in 40 regional lymph nodes (pN0) (0/40: "3", 0/13; "4", 0/5; "5", 0/1; "6", 0/5; "7", 0/3; "9", 0/0; "8a", 0/5; "11p", 0/4; "12a", 0/3; "4sb", 0/1; "1", 0/0)

Converging fold가 끌려오면 IIc로 분류해야 한다고 주장하는 분도 계십니다. Folds란 원래 함몰형 병소에 생기는 것이니까...... 그러나 flat한 EGC에도 fold가 동반되기도 합니다. Flat한 EGC 자체의 fibrotic change 때문일지도 모르겠습니다. 저는 fold와 무관하게 병소가 flat하면 EGC IIb, 약간 함몰되어 있으면 IIc로 붙이고 있습니다.

오늘의 증례는 놓치기 쉬운 병변입니다. 제법 넓더라도 flat 하면 못 볼 수 있습니다. 늘 주의합시다.
