[Gastric cancer 634 - ESD for EGC very close to previous ESD scar]

ESD for EGC close to previous ESD scar is challenging. En bloc resection of EGC and previous ESD scar was successfully done. One of the major strength of ESD is that scar lesions can be resected.

8 년 전 타 병원에서 전정부 소만에서 약간 후벽쪽 조기위암의 내시경 치료를 받은 분인데 과거 치료부에 인접하여 위암이 발견되어 의뢰되셨습니다. 두번째 사진의 화살표가 과거 scar입니다. 위암은 이 scar에 연하여 한쪽 방향으로만 자라 있었습니다. Local recur의 가능성도 있지만 저는 metachronous cancer가 과거 치료부와 가깝게 발생하였을 가능성을 더 높게 보았습니다.

이런 경우 과거 scar를 남기도 내시경 절제를 시도하면 resection margin을 확보하기 어렵습니다. 한쪽 resection margin은 포기하고 ablation을 할 수 밖에 없습니다. 그래서 다소 번거롭더라도 이번 위암과 과거 scar를 모두 포함하여 한꺼번에 ESD를 하기로 하였습니다. 한쪽에 단단한 scar가 있어서 절제가 쉽지 않았지만 성공적으로 one piece resection을 할 수 있었습니다 (시술 시간 35분).

최종 병리 결과에서도 다른 방향은 8-10mm 정도의 resection margin을 보였지만, scar가 있었던 후벽 방향은 18 mm로 넉넉한 resection margin을 보였습니다.

9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
