Single erosion can be adenoma or EGC when the shape is not round and the edge is irregular.
내시경 초심자께서 작은 single erosion을 발견하고 아래와 같이 결과를 주셨습니다.
▷ Gastric erosion
- Pre-pyloric antrum, GC side에 0.7cm sized round shape의 smooth elevated lesion이 관찰된다. 중앙에는 부정형의 erosion이 동반되어 있다.
내시경 조직검사는 adenoma with HGD로 나왔고 내시경 절제술을 하였고 모든 것이 매끄러웠습니다.
Tubular adenoma with high grade dysplasia
1. Location : antrum, posterior wall
2. Gross type : flat
3. Size of adenoma : (1) longest diameter, 4 mm (2) vertical diameter, 3 mm
4. Resection margin : negative resection margins(N)
이 증례를 보면서 느낀 점 한 가지가 있습니다. Single erosion을 발견하였고 "중앙에는 부정형의 erosion이 동반되어 있다"고 관찰하였으면 r/o adenoma 정도의 impression을 붙일 수도 있지 않았을까요? "Gastric erosion"으로만 붙인 것이 못내 아쉬웠지만.... 아직 경험이 짧으니 과감한 진단명을 붙이지 못했던 것 같습니다.
Impression을 어떻게 붙일 것인가는 매우 중요한 이슈입니다. Singel erosion으로 붙이고 조직검사 결과가 gastritis로 나오면 1-2년 후 검진 내시경을 하게 될 것입니다. r/o adenoma면 조직검사에서 adenoma가 나오지 않더라도 short-term followup을 해 보는 경우가 많습니다. r/o EGC를 붙이면 거의 항상 short-term followup을 하게 되겠지요.
어느 강도의 impression이 좋을지 고민하면서 내시경 결과를 쓰는 습관이 필요합니다. 한 마디로 '힘 조절'이 필요합니다.
