Erosion is erosion. Ulcer is ulcer. Discolorated area is discolorated area. However, the term 'erosion' is overused for everything. Careful description with adequate terminology is the beginning of endoscopic diagnosis.
위암으로 의뢰된 환자입니다. 그렇데 소견이 미란이었습니다. 사실은 EGC IIb인데요.... Flat한 discoloration인데요....
내시경계에도 유행이 있습니다. 최신 유행은 (1) 내시경 사진을 엄청 많이 찍는 일... 소위 '따발총 검사'와 (2) 병소의 특성과 무관하게 일단 미란이라고 쓰고 조직검사 하는 것입니다. 융기부도 미란, flat한 병소도 미란, 얕은 함몰형 병소도 미란, 깊은 함몰도 미란이라고 쓰는 경우를 많이 보았습니다. Impression도 없어요. 그냥 "위염, 미란"이라고만 씌여진 결과지가 너무 많습니다. 유행에 따르지 말고 원칙대로 검사합시다. 정확히 검사하고 올바르게 기술합시다.
