Small signet ring carcinoma (flat and less than 1 cm) can be treated by ESD.
위암 내시경 치료의 전통적인 적응증은 궤양이 없는, 1cm 미만의 분화형 점막암입니다. 최근에는 미분화 조직형 위암에 대한 내시경 치료에 대한 논의가 활발합니다. 아직 데이타가 충분하지 않기 때문에 1cm 미만의 signet ring cell carcinoma이면서 flat 하고 함몰부위가 없는 조기위암에 대해서만 시술하고 있습니다. Lateral margin을 깊게 확보하기 위하여 노력하고, submucosal layer를 전부 제거하기 위하여 깊게 치료하고 있습니다.
ESD: Early gastric carcinoma
1. Location : antrum, anterior wall
2. Gross type : EGC type IIb
3. Histologic type : signet ring cell carcinoma
4. Histologic type by Lauren : diffuse
5. Size of carcinoma : (1) longest diameter, 5 mm (2) vertical diameter, 2 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 14 mm, proximal 17 mm, anterior 14 mm, posterior 18 mm, deep 1200 ㎛
8. Lymphatic invasion : not identified(N)
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent
