[Gastric cancer 688 - Non-lifting in the ESD procedure]

타 병원에서 ESD를 하다가 non-lifting이 되어 시술을 중단한 상태로 의뢰된 증례입니다. 환자는 ESD를 다시 시도해 볼 것을 강력히 원하셨으나, 외부 사진과 non-lifting sign을 고려하여 ESD는 절대로 할 수 없다고 설명하고 수술을 보냈습니다. 결과를 보니 역시 수술을 보내기를 잘했다는 생각이 들었습니다.

2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : intestinal
5. Size : 1x0.7 cm
6. Depth of invasion : invades submucosa (sm3) (pT1b)
7. Resection margin: free from carcinoma; safety margin: proximal 2.9 cm, distal 5.1 cm
8. Lymph node metastasis : no metastasis in 16 regional lymph nodes (pN0)
9. Lymphatic invasion : not identified
10. Venous invasion : not identified
11. Perineural invasion : not identified
12. AJCC stage by 8th edition: pT1b N0

ESD는 환자가 원한다고 그냥 해 주는 시술이 아닙니다. 정확히 적응증이 되는 환자에서 시술하는 매우 진지한 술기입니다. 애매해도 수술을 하는 것이 좋은데 하물며 이처럼 너무나 명확히 내시경 시술 대상이 아닌 환자에서는 절대로 시술하면 안됩니다. 환자가 아무리 원하더라도...

Non-lifting으로 수술을 한 또 다른 환자입니다 (60세 여성). Well-differentiated adenocarcinoma, 3x2.8cm, deep submucosal invasion (SM3), no lymph node metastasis로 나왔습니다.
