ESD를 위하여 의뢰를 받은 환자입니다. EGC IIb인데 경계를 정확히 알 수 없었습니다. 경계를 알 수 없을 때에는 ESD를 할 수 없습니다. 생각보다 크다고 나오는 경우가 대부분이기 때문입니다. 수술을 보냈습니다.
Stomach, radical subtotal gastrectomy: Early gastric carcinoma
1. Location : middle third, Center at body and greater curvature to posterior wall
2. Gross type : EGC type IIc
3. Histologic type : tubular adenocarcinoma, moderately differentiated
4. Histologic type by Lauren : mixed
5. Size : 8.5x4.7 cm
6. Depth of invasion : invades submucosa (sm1) (pT1b)
7. Resection margin: free from carcinoma
8. Lymphatic invasion : not identified
9. Venous invasion : not identified
10. Perineural invasion : not identified
11. Lymph node metastasis : no metastasis in 33 regional lymph nodes (pN0)
12. Associated findings : gastritis cystica profunda
13. AJCC stage by 8th edition: pT1b N0
저는 IIb가 적당하다고 생각했는데 병리과에서는 IIc로 주었군요. 상관없습니다. 여하튼 ESD를 시도하지 않기를 잘 했습니다.
전략: ESD의 적응증이 확실하면 ESD를 한다. 수술의 적응증이 확실하면 수술을 한다. 약간 애매하면 ESD를 고려할 수 있지만 가급적 수술을 한다. 많이 애매하면 무조건 수술한다.
이 환자는 많이 애매했던 경우입니다. 그래서 무조건 수술!
