융기 + 함몰형 암은 조금 나쁜 결과를 가진다고 설명하고 있습니다.

Stomach, ESD : Early gastric carcinoma
1. Location : high body, lesser curvature
2. Gross type : EGC type IIb
3. Histologic type : tubular adenocarcinoma, moderately differentiated with micropapillary pattern (about 2%)
4. Histologic type by Lauren : intestinal
5. Size of carcinoma : (1) longest diameter, 14 mm (2) vertical diameter, 11 mm
6. Depth of invasion : invades mucosa (muscularis mucosa) (pT1a)
7. Resection margin : free from carcinoma(N), safety margin : distal 10 mm, proximal 9 mm, anterior 12 mm, posterior 8 mm, deep 500 ㎛
8. Lymphatic invasion : present at one focus
9. Venous invasion : not identified(N)
10. Perineural invasion : not identified(N)
11. Microscopic ulcer : absent
12. Histologic heterogeneity: absent

점막암이고 다른 인자는 모두 좋은데 lymphatic (+)인 경우에 수술을 권하는 것은 표준적인 치료입니다. 그러나 잔류암이 있다고 나오는 경우는 별로 없습니다. 저는 경험한 적이 없습니다. 그러나 권하지 않을 수도 없습니다. Lymphatic (+)는 강력한 재발 위험인자이기 때문입니다. 국가에서 운영하는 질지표에도 포함된 내용입니다.

이 환자에서도 수술이 시행되었고 residual tumor는 없었고 lymph node metastasis도 없었습니다.
